Picking up medical reports and record
Medical documents can be received by sending a written request to the Health Administration Office. The cost to receive a copy of the medical records is 20 Euros for a hard copy and 33 Euros on a USB drive.
The copy of the medical document will be ready in about 10 days from the request date and the delivery will be agreed upon. In some cases, the completion of the medical or outpatient record could require more time during the issuance process.
New times to pick up reports
In view of the current health emergency, we inform patients that starting on April 1st urgent medical reports can be picked up in person Monday to Friday from 9:00 am to 1:00 pm only by prior telephone appointment made at the following CUP number 0789 1988 898 or OPERATOR 0789 1899 9.
Medical record pick up
Pickups made in person are momentarily interrupted. In the event there is an urgent need and the indicated on-site request procedure is “pick up in person” patients can request:
- THROUGH PEC: Patients must send a request, on plain paper, to the health administration office firstname.lastname@example.org indicating their own PEC address.
- THROUGH THE MAIL: Patients must send a request, on plain paper, to the health administration office email@example.com indicating their own mailing address. In addition, 11.50€ must be paid for shipping through a bank transfer indicated in the request form.
